If you have been selected to one of our teams at Lax Elite Lacrosse Clubs you will need to complete this form. The information in this profile is required by college coaches that are interested in having you play lacrosse for them. College coaches will often contact our organization regarding a particular player. This profile is a record that we keep on file for all of our players. When we are contacted by a college coach we then send the completed form information to the coaches for recruiting purposes.
